We are looking for a compassionate and dedicated Part-Time Home Health Aide to join our team and provide outstanding support to individuals requiring assistance in their homes. Open Assignments:
- North Ridgeville: 7 AM – 7 PM, Monday – Sunday
- West Cleveland: 10 AM – 6 PM, Monday – Friday
- Lakewood: 11 AM – 3 PM, Monday, Wednesday, and Friday
- Willoughby: 12 PM – 4 PM, Monday – Friday
- Brookpark: 9 AM – 4 PM, Monday – Friday

Job Responsibilities
- Assist clients with daily tasks based on their individualized care plans, including feeding, meal preparation, linen changes, light housekeeping, and providing transportation to appointments, outdoor activities, and shopping
- Offer companionship and personal hygiene care, such as bathing, grooming, oral care, and assistance with dressing, undressing, and toileting activities
- Support clients with mobility exercises as outlined in the care plan and communicate any observed changes to the Director of Nursing
- Comply with agency policies, procedures, and protocols
- Maintain confidentiality and security of client medical information in accordance with HIPAA regulations
Job Requirements
- High school diploma or GED
- A minimum of one (1) year of documented home care assistance and/or direct care experience in an assisted living or skilled nursing facility is preferred
- Successful completion of a nursing assistant or certified home health aide program is highly desirable
- Current CPR certification and a negative TB skin test or chest x-ray
- Valid driver’s license and auto insurance (proof required)
- Ability to perform physical tasks including standing, walking, squatting, bending, kneeling, reaching, twisting, pushing, and pulling; must be able to lift up to 50 pounds
- Self-directed with minimal supervision required
- Proficient in reading, writing, speaking, and understanding English effectively
- Up-to-date vaccinations including COVID-19, Influenza, and Hepatitis B
