Job Summary:
The Youth and Adult Independent Living Supports Coach is responsible for delivering community living support services to youth (ages 5-18) and adults. This role involves providing instruction and assistance to help clients achieve their individualized service plan goals. Responsibilities include meal preparation, healthy eating initiatives, medication management, shopping, safety skills training, activities of daily living (ADLs), and facilitating community engagement while ensuring a safe and clean environment. Participation in goal evaluation and development, along with documentation completion, is a key expectation.

Essential Duties and Responsibilities:

  • Instruct and assist clients in their service plan goals and community living support programs, adhering to established objectives and intervention plans.
  • Participate in evaluations and collaboratively develop client goals, activities, and programs through team meetings.
  • Provide training in independent living skills.
  • Document daily activities as prescribed, ensuring timely completion of progress notes and incident reports.
  • Supervise medication administration and monitor health treatment applications, side effects, and health issues.
  • Transport and supervise clients during community appointments and activities, adhering to all traffic regulations.
  • Facilitate scheduled group sessions and offer meaningful one-on-one interactions to encourage independence and community inclusion.
  • Manage disbursement of client personal funds with clear recording of all transactions.
  • Intervene with individuals exhibiting inappropriate behaviors, utilizing approved intervention techniques as necessary.
  • Ensure the health and safety of clients by maintaining a clean environment, supervising activities, conducting drills, and adhering to safety guidelines.
  • Assist with shopping for food, collaborating with clients as appropriate.
  • Communicate respectfully and effectively with clients and colleagues to ensure quality service delivery.
  • Protect client confidentiality and manage information releases in compliance with regulations.
  • Document rule violations appropriately and ensure adherence to agency policies.
  • Coordinate emergency responses with on-call staff and case management.
  • Promptly notify the supervisor of any significant situations.
  • Uphold the rights of individuals served in alignment with the Michigan Mental Health Code and agency policies.
  • Support and engage in culturally competent, recovery-oriented practices, and person-centered planning to empower individuals in their recovery journeys.
  • Complete all required training and certifications in accordance with established guidelines.
  • Perform additional duties as assigned.

Required Skills/Abilities:
To excel in this role, candidates should demonstrate:

  • Strong problem-solving abilities and sound judgment for addressing both emergency and non-emergency situations.
  • Proficiency in reading, writing, and verbal communication to effectively document client objectives.
  • Human relations skills to interact effectively with diverse populations.
  • Capability to work independently.
  • Compliance with criminal record checks, driving record assessments, and recipient rights screenings.
  • A valid Michigan driver’s license, a clean driving record, and access to a licensed, insured vehicle for work-related duties.

Education and Experience:

  • High School Diploma or equivalent.
  • Experience or education working with individuals with disabilities or mental illness is preferred.
  • Background in behavioral health is desirable.

Physical Requirements:

  • Ability to squat, reach, lift, push, pull, bend, stoop, and kneel.
  • Physical capability to assist clients with lifting, transferring, personal care, and physical intervention as necessary.
  • Ability to exert up to 25 pounds of force occasionally and 15 pounds frequently, with occasional lifting up to 35 pounds.
