Job Responsibilities:

• Provide assistance with daily living activities as outlined in the client’s personalized care plan, including feeding, meal preparation, linen changes, light housekeeping, and transportation for appointments or errands.
• Deliver companionship and personal hygiene care as specified in the care plan, such as bathing, grooming, oral care, dressing, and toileting assistance.
• Facilitate clients’ mobility needs, both active and passive, as defined in the care plan and document any observed changes to the Director of Nursing.
• Adhere to all agency policies, procedures, and protocols.
• Comply with HIPAA regulations to maintain the confidentiality and security of clients’ medical information.

Job Requirements:

• High school diploma or GED.
• A minimum of one (1) year of documented experience in home care or direct care within assisted living or skilled nursing facilities is preferred.
• Preferable: Successful completion of a nursing assistant or certified home health aide program.
• Valid CPR certification and a negative TB skin test or chest x-ray.
• Must possess a valid driver’s license and auto insurance (proof of insurance is required).
• Physical capability to stand, walk, squat, bend, kneel, reach, twist, push, and pull, with the ability to lift up to 50 pounds as needed based on client mobility.
• Self-motivated with the ability to work with minimal supervision.
• Competence in adhering to HIPAA regulations, ensuring confidentiality and security of client medical information.
• Proficiency in reading, writing, speaking, and understanding English for effective communication.
• Up-to-date vaccinations, including COVID-19, Influenza, and Hepatitis B.
