JOB OVERVIEW
The Care Manager, also referred to as the “Designated Care Manager,” is dedicated to providing the highest quality of care and services to a consistent group of residents and their families within our assisted living, long-term care, and reminiscence neighborhoods. The Care Manager delivers hands-on care and both physical and emotional support as detailed in each resident’s Individualized Service Plan (ISP), all while maintaining a safe and comfortable home-like environment. This role embodies the mission of Sunrise Senior Living: “to champion quality of life for all seniors,” in compliance with federal, state/provincial, and local laws as well as Sunrise standards, policies, and procedures.

RESPONSIBILITIES & QUALIFICATIONS

Essential Duties

The unique responsibilities of this position include, but are not limited to, the following essential functions:

Resident Care

  • Review and document entries in the Daily Log to stay informed about pertinent information regarding residents.
  • Participate in the formulation and monthly updates of Individualized Service Plans (ISPs).
  • Take responsibility for a designated group of residents during each shift, ensuring their safety and wellbeing.
  • Observe, report, and document any changes in residents’ conditions including skin integrity, behavior, alertness, weight, diet, and activity participation.
  • Attend daily crossover meetings led by the lead care manager.
  • Inform supervisors and the resident care director of any increased care needs of residents.
  • Respond promptly to security systems and resident call bells, taking appropriate actions as necessary.
  • Greet guests, family members, residents, and team members in a cordial manner.
  • Handle telephone inquiries in a timely, courteous, and professional manner.
  • Assist with continence management while ensuring proper sanitation practices in resident suites and common restrooms.
  • Communicate effectively with families and act as a resource when needed.
  • Support residents in maintaining independence and promoting dignity in alignment with Resident Rights and Sunrise Principles of Service.
  • Participate in and facilitate activities of daily living (ADLs), encouraging residents to engage in activities as indicated on their ISPs.
  • Engage residents in life skills and enrichment activities throughout the day in reminiscence.
  • Approach every interaction with empathy, being mindful of each resident’s individual communication styles and basic human needs.
  • Foster an atmosphere of warmth, personal interest, and tranquility while adhering to established safety protocols.
  • Maintain high standards of cleanliness, hygiene, and health.
  • Host and engage residents in daily activities.

Risk Management and General Safety

  • Collaborate with the community team to ensure compliance with national/provincial regulations regarding occupational health and safety, promoting Risk Management programs and policies.
  • Adhere to safety procedures, including the use of Personal Protective Equipment (PPE) and fire safety protocols.
  • Report any accidents or incidents immediately.
  • Notify relevant personnel of unsafe conditions or hazardous equipment without delay.
  • Ensure a safe environment by keeping pathways clear and complying with fire codes.
  • Understand and practice safe handling and disposal methods for potentially hazardous materials.
  • Report any occupational exposure to infectious materials or hazardous substances.
  • Appropriately store and monitor oxygen tanks.

Housekeeping and Laundry Services

  • Maintain cleanliness within assigned resident rooms, including routine tasks such as making beds and managing waste.
  • Wash and manage residents’ laundry as specified in their ISPs.
  • Prepare dining room linens as needed.
  • Complete assigned housekeeping tasks and ensure common areas are kept tidy.

Dining Service

  • Serve meals with attention to creating a pleasant dining experience.
  • Assist with dining room setup and cleanup as directed.
  • Participate in pre-meal briefings.
  • Adhere to prescribed diets as per ISPs.
  • Note and document any changes in residents’ dining habits.
  • Provide room service when required, maintaining compliance with safe food handling practices.

Specific Responsibilities for the Reminiscence Neighborhood Care Manager

  • Engage residents in activities that promote life skills as outlined in their ISP.
  • Encourage participation in social activities, including afternoon gatherings.
  • Incorporate the concept of ‘Creating Pleasant Days’ into daily routines.
  • Include each resident’s individual life skills in their daily activities.
  • Use sensory experiences to enrich residents’ daily lives.
  • Assist in composing monthly letters to families.

Training and Contributing to Team Success

  • Collaborate with team members to achieve shared goals.
  • Demonstrate adherence to our Team Member Credo in daily interactions.
  • Commit to serving residents and guests through our Principles of Service.
  • Participate in engagement initiatives and improvement planning workshops.
  • Attend regular team meetings and training sessions as directed.
  • Comply with infection control policies and training requirements, ensuring adherence to state/provincial regulations.
  • Perform additional duties as assigned.

Experience and Qualifications

  • Minimum age of 18 years.
  • Prior experience with seniors is preferred.
  • A genuine desire to serve and care for seniors.
  • Ability to act in the best interests of residents.
  • Compliance with driving requirements for Sunrise team members if applicable.
