JOB OVERVIEW
The Care Manager, also referred to as the Designated Care Manager, is tasked with delivering the highest level of quality care and services to a designated group of residents and their families within our assisted living, long-term care, and reminiscence neighborhoods. This role encompasses providing hands-on care and emotional support tailored to each resident’s Individualized Service Plan (ISP), while fostering a safe and home-like environment. The Care Manager is dedicated to fulfilling our mission at Sunrise Senior Living: “to champion quality of life for all seniors,” in alignment with all applicable regulations, standards, and policies.
RESPONSIBILITIES & QUALIFICATIONS
Essential Duties

Resident Care
- Review and document pertinent resident information in the Daily Log.
- Participate in the development and monthly updates of ISPs.
- Manage designated assignments and take responsibility for a group of residents during shifts, ensuring their safety through regular check-ins.
- Observe, report, and document changes in residents’ conditions regarding various health indicators.
- Attend daily crossover meetings led by the Care Manager.
- Notify supervisors of residents with increased care needs and report any notable changes.
- Respond promptly to security systems and resident call requests, taking necessary actions.
- Engage with guests, family members, residents, and team members warmly and professionally.
- Assist in continence management while ensuring sanitation practices are upheld.
- Maintain communication with families, serving as a resource as needed.
- Support residents’ independence and promote their dignity.
- Facilitate activities of daily living (ADLs) and encourage participation as indicated in the ISP.
- Engage residents in life enrichment activities throughout the day.
- Maintain a comforting atmosphere of warmth and positivity.
- Follow established safety protocols consistently.
- Uphold hygiene and health standards through routine cleanliness practices.
- Actively participate in resident activities.
Risk Management and General Safety
- Collaborate with community teams to maintain compliance with occupational health and safety regulations.
- Adhere to safety procedures, including the use of Personal Protective Equipment (PPE) and reporting any incidents or hazards immediately.
- Ensure a safe environment by keeping areas clear of obstacles and complying with fire safety codes.
Housekeeping and Laundry Services
- Clean and maintain residents’ rooms and common areas as assigned.
- Handle residents’ laundry in accordance with the ISP and other housekeeping tasks as needed.
Dining Service
- Serve meals and promote a pleasant dining experience.
- Assist with dining room setup and cleanup while following dietary guidelines.
- Document changes in residents’ dining habits and maintain adherence to food safety standards.
Specific Responsibilities for the Reminiscence Neighborhood Care Manager
- Engage residents in life skills and enriching activities as outlined in their ISP.
- Incorporate the concept of ‘Creating Pleasant Days’ into daily interactions.
- Enhance daily routines with multi-sensory experiences.
- Facilitate regular communication through letters with residents and their families.
Training and Contributing to Team Success
- Collaborate with team members towards common goals and demonstrate our Team Member Credo in everyday interactions.
- Attend regular meetings and training sessions to stay informed and compliant with Sunrise standards.
- Participate in engagement surveys and contribute to improvement planning.

Experience and Qualifications

- Demonstrates a genuine desire to serve and care for seniors.
- Decision-making skills prioritized in the best interests of residents.
- Compliance with Sunrise vehicle guidelines, if applicable.
