JOB OVERVIEW
The Care Manager, also referred to as the “Designated Care Manager,” is tasked with delivering the highest standard of quality care and services to a consistent group of residents and their families within our assisted living, long-term care, and reminiscence neighborhoods. This role involves providing hands-on care and holistic support as outlined in each resident’s Individualized Service Plan (ISP), while ensuring a safe and comfortable home-like environment. The Care Manager is committed to embodying the mission of Sunrise Senior Living, which is to champion quality of life for all seniors, adhering to all federal, state/provincial, and local regulations, as well as Sunrise standards, policies, and procedures.
RESPONSIBILITIES & QUALIFICATIONS
Essential Duties

Resident Care
- Review the Daily Log to document and assimilate pertinent resident information.
- Participate in the development and monthly updates of Individualized Service Plans (ISPs).
- Oversee designated assignments and manage a consistent group of residents during each shift, ensuring frequent physical check-ins.
- Observe, report, and document any changes in residents’ conditions, including skin integrity, behavior, alertness, weight, diet, and participation in activities.
- Attend daily crossover meetings led by the lead care manager.
- Notify supervisors and the resident care director of any increased care needs or changes in condition.
- Promptly respond to security system alerts and resident call bells.
- Greet guests, family members, and fellow team members in a welcoming manner.
- Answer phone calls courteously and professionally.
- Assist with continence management and ensure proper disposal of all products to maintain sanitation.
- Maintain open communication with families, serving as a resource as needed.
- Support residents in maintaining independence and physical safety while promoting their dignity according to resident rights and Sunrise Principles of Service.
- Lead and assist residents with activities of daily living (ADLs) and engage them in activities highlighted in their ISPs.
- Facilitate life skills and enrichment activities throughout the day in reminiscence.
- Approach each resident with empathy, respecting their unique communication patterns and fundamental human needs.
- Foster an atmosphere of warmth and personal interest while maintaining a calm environment.
- Adhere to established safety regulations at all times.
- Practice stringent cleanliness, hygiene, and health measures consistently.
- Coordinate and participate in daily activities with residents.
Risk Management and General Safety
- Collaborate with the community team to comply with national/provincial occupational health and safety regulations, promote Risk Management programs, and adhere to safety rules.
- Implement safety procedures at all times, including the use of Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout practices.
- Report all incidents or accidents immediately.
- Address unsafe or hazardous conditions promptly.
- Ensure that all hazards, including cords and equipment, are stored properly and do not obstruct exits or fire safety measures.
- Understand and follow protocols related to exposure to bloodborne pathogens and hazardous materials.
- Safely store and monitor oxygen tanks.
Housekeeping and Laundry Services
- Maintain cleanliness in assigned resident rooms, including making beds, disposing of trash, and spot cleaning when necessary.
- Manage residents’ laundry as outlined in ISPs.
- Wash and fold dining room linens and maintain a clean dining area.
Dining Service
- Serve meals in the dining room and assist with dining room operations as required.
- Ensure a pleasant dining experience for all residents and guests.
- Assist with dining room setup and cleanup.
- Adhere to residents’ dietary requirements as specified in ISPs and the Confidential Diet Board/Chart.
- Document any changes in residents’ dining habits in the daily log.
- Respond effectively to the dining needs of residents while upholding Sunrise hospitality standards.
- Provide room service as needed and observe proper food handling protocols.
Specific Responsibilities for the Reminiscence Neighborhood Care Manager
- Engage residents in life skills and enriching activities as indicated in their ISPs.
- Encourage participation in afternoon social activities.
- Integrate the concept of “Creating Pleasant Days” into residents’ routines.
- Incorporate residents’ individual life skills into daily interactions.
- Utilize a variety of multi-sensory experiences throughout the day.
- Facilitate monthly correspondence with residents and their families.
Training and Contributing to Team Success
- Collaborate as a team member to achieve shared goals.
- Demonstrate our Team Member Credo in daily interactions.
- Commit to serving residents and guests by upholding our Principles of Service.
- Participate in engagement programs and activities, including team member engagement surveys and planning workshops.
- Attend regular meetings as directed.
- Participate in training conducted by Assisted Living Coordinators, Resident Care Coordinators, or Reminiscence Coordinators.
- Adhere to all infection control protocols and comply with training requirements per state or local regulations.

Experience and Qualifications

- Must be at least 18 years of age.
- Previous experience working with seniors is preferred.
- A genuine desire to serve and care for seniors.
- Ability to make informed choices and act in the best interests of residents.
- All team members driving a Sunrise vehicle must understand safety and regulatory compliance as outlined in the job description.
