Job Overview
The Care Manager/Designated Care Manager is entrusted with delivering the highest quality of care and services to a designated group of residents and their families within our assisted living, long-term care, and reminiscence neighborhoods. This role involves providing hands-on care, along with physical and emotional support, as stipulated in each resident’s Individualized Service Plan (ISP), all while fostering a safe and comfortable, home-like atmosphere. The Care Manager is also responsible for embodying Sunrise Senior Living’s mission: "to champion quality of life for all seniors," adhering to federal, state/provincial, and local regulations as well as Sunrise’s internal standards, policies, and procedures.

Responsibilities & Qualifications

Essential Duties

Resident Care

  • Review and document relevant information in the Daily Log to stay informed about residents.
  • Participate in the development and monthly updates of ISPs.
  • Manage designated assignments and oversee a specific group of residents during each shift, ensuring their safety and well-being.
  • Observe, report, and document any changes in residents’ conditions, including skin integrity, behavior, alertness, weight, diet, and activity participation.
  • Attend daily Cross Over meetings facilitated by the lead Care Manager.
  • Notify the supervisor and resident care director of any increased care needs.
  • Respond promptly to security and resident call systems, taking appropriate action.
  • Welcome guests, family members, residents, and team members with professional courtesy.
  • Manage communication with families, acting as a resource when needed.
  • Assist residents in maintaining independence and safety while promoting dignity in accordance with Resident Rights and Sunrise Principles of Service.
  • Guide residents in activities of daily living (ADLs) as laid out in their ISPs and encourage participation in organized activities.
  • Engage residents in life enrichment activities, specifically within the reminiscence neighborhood.
  • Approach each resident with empathy and adaptability, acknowledging their communication styles and unique histories.
  • Maintain a warm environment that fosters personal engagement while upholding safety regulations and standards.
  • Regularly practice cleanliness and hygiene, adhering to Standard Care Precautions.

Risk Management and General Safety

  • Collaborate with the community team to ensure compliance with national/provincial occupational health and safety regulations, while promoting Risk Management policies.
  • Adhere to safety procedures, utilizing Personal Protective Equipment (PPE) and being familiar with emergency protocols.
  • Report accidents, incidents, and hazardous conditions immediately.
  • Ensure that all safety equipment and areas remain free from obstruction and comply with fire codes.

Housekeeping and Laundry Services

  • Maintain cleanliness in residents’ rooms, including making beds and managing trash.
  • Manage residents’ laundry in accordance with ISPs.
  • Maintain communal areas in pristine condition at all times.

Dining Service

  • Serve meals and assist in dining room operations, ensuring a pleasant dining experience.
  • Adhere strictly to residents’ dietary needs per ISPs while keeping detailed records of any changes in dining habits.
  • Promote hospitality and service excellence during meal service and respond to residents’ dining needs.

Specific Responsibilities for the Reminiscence Neighborhood Care Manager

  • Engage residents in life skills and enriching activities consistent with their ISPs.
  • Foster an environment that creates pleasant daily experiences for residents.
  • Incorporate sensory elements into residents’ daily routines.

Team Collaboration and Development

  • Actively participate as a team member, contributing towards collective goals.

  • Attend regular meetings and training sessions to enhance knowledge and compliance with infection control techniques and Sunrise standards.
