JOB OVERVIEW
The Care Manager, also referred to as the “Designated Care Manager,” is tasked with delivering the highest standard of care and services to a consistent group of residents and their families within our assisted living, long-term care, and reminiscence neighborhoods. This role provides hands-on care and both physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP), while ensuring a safe and comfortable, homelike environment. The Care Manager is responsible for embodying the Mission of Sunrise Senior Living: "to champion quality of life for all seniors," in accordance with federal, state/provincial, and local laws and regulations, as well as Sunrise Senior Living standards, policies, and procedures, to foster the highest degree of care and service quality for our residents.
RESPONSIBILITIES & QUALIFICATIONS
Essential Duties

The specific responsibilities for this role include, but are not limited to, the following essential functions:
Resident Care
- Review, annotate, and initial Daily Log to document and remain informed about relevant resident information.
- Collaborate in the development and monthly updates of ISPs.
- Assess designated assignments.
- Be accountable for a designated group of residents during each shift, knowing their locations and conducting physical checks throughout the shift.
- Observe, report, and document any changes in residents’ conditions, including skin integrity, behavior, alertness, weight, diet, and engagement in activities.
- Participate in daily Cross Over meetings led by the lead care manager.
- Notify the supervisor and resident care director of any resident with increased care needs.
- Communicate any changes in residents’ conditions to the supervisor promptly.
- Respond promptly to security system alerts and resident call bells, taking appropriate actions such as resetting call bells.
- Welcome guests, family members, residents, and team members warmly.
- Handle phone calls in a courteous and professional manner, directing or responding as necessary.
- Assist with continence management and properly dispose of all continence products to ensure the sanitation of resident suites and community restrooms.
- Serve as a resource for families as needed.
- Enable residents to maintain independence and physical safety while upholding their dignity in accordance with Resident Rights and Sunrise Principles of Service.
- Assist residents with activities of daily living (ADLs) and invite, encourage, and assist them to participate in activities as indicated in their ISPs.
- Engage residents in life skills and other enrichment activities throughout the day in reminiscence settings.
- Approach each resident with empathy, remaining attentive to their unique communication patterns and basic human needs.
- Foster an atmosphere of warmth, personal interest, and positivity while maintaining a calm environment.
- Adhere to established safety regulations at all times.
- Consistently practice good Standard Care Precautions related to cleanliness, hygiene, and health.
- Facilitate and engage in daily activities with the residents.
Risk Management and General Safety
- Collaborate with the community team to ensure adherence to national/provincial regulations relevant to occupational health and safety, promoting Risk Management programs and policies and complying with safety rules and regulations.
- Always practice safety procedures, including the use of Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
- Report all accidents and incidents immediately.
- Immediately report any unsafe or hazardous conditions or equipment.
- Ensure that equipment and hazards are kept clear of exits and in compliance with fire codes.
- Understand and follow proper procedures for attending to and disposing of, and the potential exposure to, bloodborne pathogens, bodily fluids, and hazardous materials.
- Promptly report any occupational exposures to blood, body fluids, or hazardous materials.
- Ensure that oxygen tanks are stored safely, exchanged when empty, and monitored for compliance.
Housekeeping and Laundry Services
- Maintain and clean residents’ rooms as assigned, including tasks like making beds, emptying trash, and organizing clothes.
- Manage residents’ laundry as specified in ISPs and as needed.
- Wash and fold dining room linens and napkins.
- Complete housekeeping tasks as assigned.
- Keep common areas clean and tidy at all times.
Dining Service
- Serve meals in the dining room and fulfill assigned dining room responsibilities.
- Promote a pleasant dining experience during all meals.
- Assist with dining room setup and cleanup as directed.
- Participate in pre-meal meetings.
- Follow residents’ dietary requirements as outlined in ISPs and the Confidential Diet Board/Chart.
- Document any changes in residents’ dining habits in the daily log.
- Respond to the dining needs of residents and guests while maintaining Sunrise’s hospitality and service standards.
- Provide room service delivery as required.
- Adhere to safe food handling practices in line with universal care precautions.
Specific Responsibilities for the Reminiscence Neighborhood Care Manager
- Engage residents in life skills and other enriching activities as indicated in their ISPs and demographic profiles.
- Encourage residents to attend afternoon social events.
- Incorporate the principle of ‘Creating Pleasant Days’ into residents’ daily routines.
- Integrate individual residents’ life skills into their daily experiences.
- Blend various multi-sensory experiences into daily activities for residents.
- Participate in monthly correspondence with residents and their families.
Training and Contributing to Team Success
- Function as a committed team member, working towards common goals.

- Dedicate yourself to serving our residents and guests through our Principles of Service.
- Contribute to overall engagement programs and processes, including participating in team member engagement surveys and improvement workshops.
- Attend regular team meetings, including Town Hall, Department Team, and Cross Over meetings as directed by the Supervisor/Department Coordinator.
- Engage in ongoing training provided by assisted living coordinators through mini-modules.
- Comply with infection control protocols and the proper placement and removal of biohazard containers as per policies and procedures.
- Ensure compliance with assigned required training and all relevant training mandated by state/provincial authorities applicable to this role.
- Perform other duties as assigned.

Experience and Qualifications

- Ability to make decisions in the best interest of residents.
- If applicable, all Sunrise team members who drive a Sunrise vehicle must understand and sign the Driver job description and key safety duties.
