Job Overview:
The Care Manager, also referred to as the “Designated Care Manager,” is entrusted with delivering the highest quality of care and services to a designated group of residents and their families within our assisted living, long-term care, and reminiscence neighborhoods. This role encompasses providing hands-on care and both physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP), all while fostering a safe, comfortable, and homelike environment. Responsibilities & Qualifications:
Essential Duties:

Resident Care
- Review, annotate, and initial the Daily Log to stay informed about pertinent resident information.
- Participate in the development and monthly updates of Individualized Service Plans (ISPs).
- Oversee designated assignments.
- Be responsible for a specific group of residents during your shift, ensuring their whereabouts and conducting routine checks.
- Observe, report, and document any changes in the residents’ conditions, including skin, behavior, alertness, weight, diet, and participation in activities.
- Attend daily Cross Over meetings led by the Care Manager.
- Notify the supervisor and Resident Care Director of any increase in a resident’s care needs.
- Communicate any changes in resident conditions to the supervisor.
- Respond swiftly to security systems and resident call bells, taking appropriate actions as necessary.
- Welcome guests, family members, residents, and team members with warmth.
- Professionally handle phone calls, directing or responding in a timely manner.
- Assist with continence management, ensuring proper disposal of products to maintain sanitation standards.
- Serve as a resource for families as needed.
- Uphold residents’ independence, physical safety, and dignity in accordance with Resident Rights and Sunrise Principles of Service.
- Engage residents in activities of daily living (ADLs), encouraging participation as per the ISP.
- Support residents in life skills and other enriching activities throughout the day within the reminiscence neighborhood.
- Approach each resident with empathy, being mindful of their unique communication patterns and fundamental human needs.
- Foster a warm, engaging, and soothing environment for residents.
- Adhere to all safety regulations consistently.
- Maintain cleanliness, hygiene, and health standards systematically.
- Host and participate in daily activities with residents.
Risk Management and General Safety
- Collaborate with the community team to ensure compliance with national/provincial regulations regarding occupational health and safety, promoting Risk Management policies.
- Observe safety procedures at all times, including the use of Personal Protective Equipment (PPE).
- Report any accidents or incidents immediately.
- Alert supervisors to any unsafe or hazardous conditions.
- Ensure that cords, carts, equipment, and other hazards are managed to comply with fire safety codes.
- Understand and apply proper methods for handling blood-borne pathogens and other potentially hazardous materials.
- Immediately report any occupational exposure to hazardous materials.
- Ensure oxygen tanks are stored correctly, exchanged when necessary, and monitored for prescribed levels.
Housekeeping and Laundry Services
- Regularly clean and maintain assigned resident rooms, including tasks such as making beds and disposing of trash.
- Handle resident laundry as outlined in the ISP.
- Maintain cleanliness in common areas consistently.
Dining Service
- Serve meals in the dining room and assist with related tasks as needed.
- Foster an enjoyable dining experience for all residents.
- Support dining room setup and cleanup efforts as assigned.
- Participate in pre-meal meetings.
- Adhere to residents’ dietary needs as outlined in ISPs.
- Document any changes in residents’ dining habits.
- Attend to the dining needs of residents and guests, upholding Sunrise standards of hospitality and service.
- Provide room service delivery as necessary.
- Ensure safe food handling practices at all times.
Specific Responsibilities for the Reminiscence Neighborhood Care Manager
- Engage residents in life skills and other enriching activities as specified in the ISP.
- Encourage participation in afternoon social events.
- Infuse the concept of ‘Creating Pleasant Days’ into daily routines.
- Incorporate residents’ life skills into their everyday activities.
- Provide a variety of multi-sensory experiences throughout the day.
- Assist in monthly correspondence with residents and their families.
Training and Contributing to Team Success
- Actively participate as a collaborative team member dedicated to achieving team goals.

Experience and Qualifications:

- Genuine desire to care for and serve seniors.
- Ability to make decisions in the best interest of residents.
