As a Care Manager at Sunrise, you will play a crucial role in delivering the highest standard of quality care and services to a designated group of residents and their families within our assisted living and reminiscence neighborhoods.
Responsibilities:
- Develop meaningful relationships with a specific group of seniors and their families by providing assistance with activities of daily living, addressing individual care needs, and understanding their unique preferences and personalities.
- Communicate any changes in residents’ conditions to management and recommend necessary adjustments in care and service.
- Assist residents with life skills and engage them in life-enriching activities as outlined in their individual profiles.
- Incorporate a variety of multi-sensory experiences into residents’ daily routines.
- Participate in the development and monthly updates of Individualized Service Plans (ISP).
- Maintain awareness of your designated group of residents during your shift, ensuring their safety and well-being through regular physical checks.
- Collaborate with the community team to ensure compliance with OSHA regulations and promote Risk Management programs and policies, maintaining adherence to safety rules and regulations.
- Attend to the dining needs of residents and guests while upholding Sunrise’s hospitality and service standards.
- Invite, encourage, and assist (IEA) residents in life skills and other enriching activities as specified in their ISPs and demographic profiles.
- Commit to serving our residents and guests in alignment with our Principles of Service.
Qualifications:
- A dedication to and passion for serving seniors, complemented by excellent customer service skills.
- A positive attitude, flexibility in performing various duties, and an ability to work collaboratively within a team environment are essential for success.
- A High School diploma or GED is accepted and may be required based on state regulations.
- In states where applicable, candidates must maintain relevant certifications.
- Candidates must be at least 18 years of age.
- Previous experience working with seniors is preferred.
- Ability to make informed choices and decisions in the best interest of residents.
- Strong written and verbal communication skills, along with a clear level of understanding.
- Competent organizational and time management skills.
- Demonstrated good judgment, problem-solving, and decision-making abilities.
