Open Assignments:

  • North Ridgeville: 7 AM – 7 PM, Monday through Sunday
  • West Cleveland: Monday through Friday, 10 AM – 6 PM
  • Lakewood: Monday, Wednesday & Friday, 11 AM – 3 PM
  • Brookpark: Monday through Friday, 9 AM – 4 PM

Job Responsibilities:

  • Assist clients with daily tasks as outlined in their personalized care plan, including but not limited to: feeding, meal preparation, linen changes, light housekeeping, and providing transportation for medical appointments, outdoor activities, and shopping.
  • Offer companionship and personal hygiene care, adhering to the individualized care plan, including bathing, grooming, oral care, dressing, toileting activities, and assisting with measurements as necessary.
  • Support clients with active and passive mobility exercises as defined in the care plan and document any observed changes to the Director of Nursing.
  • Comply with the policies, procedures, and protocols of the agency.
  • Maintain confidentiality and security of clients’ medical information in accordance with HIPAA laws.

Job Requirements:

  • High school diploma or GED.
  • A minimum of one (1) year of documented experience in home care assistance and/or direct care within assisted living or skilled nursing facilities is preferred.
  • Certification in a nursing assistant or certified home health aide program is desirable.
  • Current CPR certification and a recent negative TB skin test or chest x-ray.
  • Valid driver’s license and auto insurance (proof required).
  • Ability to perform physical tasks, including standing, walking, squatting, bending, kneeling, reaching, twisting, pushing, and pulling. Must be able to lift up to 50 pounds.
  • Self-directed with the capacity to work independently with minimal supervision.
  • Proficient in reading, writing, speaking, and understanding English, with effective communication skills.
  • Up-to-date vaccinations, including COVID-19, Influenza, and Hepatitis B.
