Job Responsibilities:
- Provide support with daily tasks as outlined in the client’s personalized care plan, including but not limited to feeding, meal preparation, linen changes, light housekeeping, transportation to appointments, and outdoor activities.
- Offer companionship, as well as personal and hygiene care per the client’s care plan, including bathing, grooming, oral care, dressing, undressing, and toileting assistance.
- Assist clients with mobility activities defined in their care plan and document observed changes to the Director of Nursing.
- Comply with agency policies, procedures, and protocols.
- Uphold HIPAA regulations to maintain the confidentiality and security of clients’ medical information.
Job Requirements:
- High school diploma or GED.
- At least one year of documented experience in home care assistance or direct care in an assisted living or skilled nursing setting is preferred.
- Preferred: Completion of a nursing assistant or certified home health aide program.
- Certified in CPR and possess a negative TB skin test or chest X-ray.
- Valid driver’s license and auto insurance (proof of insurance required).
- Physical capability to lift up to 50 pounds, as well as stand, walk, squat, bend, kneel, reach, twist, push, and pull as needed.
- Self-directed with the ability to work independently with minimal supervision.
- Proficient in reading, writing, speaking, and understanding English to communicate effectively.
- Up-to-date vaccinations, including COVID-19, Influenza, and Hepatitis B.
