Job description:
Under the supervision of the Children’s Special Health Care Services/Child Lead, Nurse Family Partnership, or Maternal Infant Health Program Supervisor, the Public Health Nurse (PHN) is responsible for identifying and addressing community health needs through the planning, implementation, and evaluation of public health nursing services aimed primarily at families below the income threshold for the Women, Infants, and Children (WIC) program. The PHN conducts comprehensive public health assessments and provides nursing services to children, expectant parents, families, and the broader community, operating under one of the following programs: Children’s Special Health Care Services, Child Lead, Maternal Infant Health Program, or Nurse Family Partnership. Additionally, the PHN may engage in specific initiatives such as the Fetal Infant Mortality Review (FIMR) and Child Death Review (CDR).
Essential Functions:
– Conduct home, school, office, and community visits to assess the physical, emotional, social, and environmental needs of individuals and families, focusing on healthy development throughout the life course.
– Build rapport with clients to foster autonomy, self-esteem, and problem-solving skills.
– Enhance health literacy in the community by developing evidence-based health education materials and leading group teaching sessions on topics such as reducing lead exposure, childbirth, and car seat safety.
– Collaborate with interdisciplinary teams involved in the circles of care for clients.
– Assist in the identification and establishment of goals that are specific, measurable, achievable, relevant, and time-bound to enhance health and wellness.
– Implement personalized care plans that target areas such as lead exposure reduction, parenting support, postpartum care, nutrition, child healthcare, family planning, and special health needs.
– Continuously monitor progress towards goals, reviewing and adjusting care plans as necessary.
– Facilitate needs-based referrals to community agencies and resources.
– Serve a diverse clientele, including families who predominantly speak languages other than English.
– Participate in Continuous Quality Improvement projects as required by specific programs.
– Engage in provider education and outreach initiatives.
– Utilize data to guide appropriate interventions, ensuring optimal health outcomes for clients.
– Collaborate with State of Michigan staff to deliver necessary services and comply with all program requirements.
– Adhere to best practices in nursing documentation, ensuring all nursing activities are recorded promptly.

– Conduct case extraction services for FIMR and/or CDR, which may include contacting providers for medical record reviews, interviewing community service representatives, or engaging with families who have experienced infant or child loss.
– Present case information to FIMR and/or CDR committees to inform future recommendations for preventing infant and child deaths.
– Participate in committees focused on developing death prevention strategies.
– Engage in public policy education and advocacy under program guidance.
– Collaborate and communicate professionally with various community organizations, such as the Local Leadership Group or Great Start Coalition.
– Serve on committees within their designated program or Ingham County to promote positive organizational development.
– Educate nursing students about public health nursing through shadowing opportunities, group educational events, and other relevant initiatives.
– Ensure that client records are secure, confidential, and maintained in accordance with Health Department policies and HIPAA regulations.
– Be prepared to perform duties similar to those outlined in this job description during public health emergencies.

Employment Qualifications:
Education: A Bachelor’s Degree in Nursing is required.
Licensure: A current, unrestricted license to practice as a Registered Nurse in the State of Michigan is mandatory.
