Under the guidance of the Wound Care Center Clinical Nurse Manager/Clinical Coordinator, the Registered Nurse (RN) plays a crucial role in delivering patient care and managing intake and discharge processes for Wound Care Center patients as scheduled. Depending on the Wound Care Center setup, the RN may also serve as a Case Manager for a designated group of patients.
All Healogics employees are required to perform their job responsibilities in accordance with company policies, hospital protocols, and applicable regulations set forth by accrediting organizations and the Centers for Medicare and Medicaid Services (CMS).
Essential Functions/Responsibilities:
- Deliver patient care to Wound Care Center patients and manage intake and discharge processes in accordance with hospital and nursing standards.
- Assist in the vein clinic as assigned, upon completion of Healogics Vein Clinic training.
- Serve as Case Manager for a group of patients, including:
- Conducting initial nursing assessments and communicating findings with the assigned physician.
- Assessing wound stages and measurements, documenting care plans, evaluating patient status concerning desired outcomes, and identifying situations requiring intervention.
- Engaging in quality improvement initiatives, providing education to patients or caregivers, and advocating for patients in the delivery and coordination of care.
- Performing hyperbaric oxygen therapy duties as assigned, after completing Healogics hyperbaric training.
- Assuming hyperbaric safety director responsibilities upon completion of required training.
- Collaborating with other healthcare providers, Wound Care Center Providers, the Program Director, and the Medical Director regarding clinic and patient needs.
- Functioning as a Documentation Assistant (scribe) as per Healogics policy.
- Performing additional duties as required.
Required Education, Experience, and Credentials:
- Associate Degree in Nursing (ADN).
- Current RN license in the state of practice, to be maintained throughout employment.
- Bachelor of Science in Nursing (BSN) preferred.
- Medical Surgical, Home Health, or Wound Care Certification preferred.
- Wound care experience preferred.
Required Knowledge, Skills, and Abilities:
- Demonstrated proficiency in Basic Life Support (BLS).
- Strong organizational, prioritization, and time-management skills.
- Fundamental understanding of regulatory and accrediting requirements for healthcare organizations.
- Excellent interpersonal, listening, and communication skills, with the ability to articulate complex technical information in layman’s terms.
