The Telehealth Registered Nurse (RN) acts as a patient care liaison, coordinating services throughout the continuum of care to enhance quality, minimize avoidable readmissions and emergency visits, and reduce healthcare costs. This role involves managing patients remotely, coordinating care, and liaising with physicians as appropriate. The Telehealth RN is essential in preventing rehospitalizations and Emergency Room visits, facilitating care within the community through effective communication with physician offices, community resources, patients, and their families. The role employs innovative workflows to enhance patient care and wellness.

Collaboration with healthcare providers is key to achieving optimal patient outcomes, ensuring comprehensive coordination and education for all stakeholders involved in patient care. Additional responsibilities may be assigned as needed.

This position adheres to the Michigan Public Health Code Act 368 of 1978, Article 15 (Health Occupations), Part 172 (Nursing), as well as the ANA Registered Nurse Standards and the Holland Hospital Professional Nursing Practice Handbook.

Pay Range: $28.71 – $48.77/hour
Employment Type: Part-Time, 40 hours every 2 weeks
Schedule: Five 8-hour shifts on varied days every 2 weeks

Requirements:

  • Graduate of an accredited Registered Nursing Program
  • Current Michigan RN License (or to be completed by the end of orientation)
  • Current BLS for the Healthcare Provider (or to be completed within 60 days of hire)

Telehealth Patient Oversight

  • Conduct daily clinical oversight of all telehealth patients, including monitoring trends, reporting to physicians, and coordinating follow-up care as necessary.
  • Advocate for interventions that prevent rehospitalizations and Emergency Room visits, identifying and facilitating the identification of appropriate telehealth patients.
  • Educate patients and internal/external stakeholders about telehealth services.

Planning and Coordination

  • Collaborate with an interdisciplinary team, including Care Management, Readmission Prevention, Pharmacy, Home Health, and Physician Practices, to maximize clinical quality outcomes.
  • Serve as a liaison for case updates and work collaboratively with post-acute providers.

Assessment and Patient Care

  • Notify provider offices of Telehealth enrollment through appropriate communication channels, such as fax, EMR, or phone calls.
  • Perform telephonic assessments with patients, addressing symptom-specific questions related to remote monitoring data and vital sign instability, while collaborating with patients, families, and providers to establish follow-up visits, assess potential needs, identify readmission risks, and resolve issues.
  • Triage necessary levels of contact based on remote patient monitoring data and patient stability. Collaborate with home health staff, delegate tasks, and provide general supervision to telehealth assistants. Offer advice to non-RN Home Health staff regarding recommended actions for vital sign concerns.
  • Complete documentation in RPM EMR and other EMRs for effective communication regarding vital sign concerns, upcoming labs, follow-up appointments, cardiac rehab hand-offs, and discharge planning.

Programmatic Improvement

  • Participate in investigations to determine the root causes of readmissions.
  • Stay informed about processes and contribute suggestions and plans for improvements.
