Job Overview
The Care Manager, referred to as the “Designated Care Manager,” is charged with delivering the highest quality of care and services to a dedicated group of residents and their families within our assisted living, long-term care, and reminiscence neighborhoods. This role involves providing hands-on care and both physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP), while ensuring a safe and comfortable, home-like environment. As a Care Manager, you will embody Sunrise Senior Living’s mission to “champion quality of life for all seniors,” adhering to all relevant federal, state, and local regulations, as well as Sunrise’s standards and policies to uphold the highest degree of service.

Responsibilities & Qualifications

Resident Care

  • Thoroughly review, document, and initial the Daily Log to stay informed about pertinent resident information.
  • Participate in the development and monthly updates of ISPs.
  • Manage designated assignments responsibly.
  • Ensure consistent oversight of a specific group of residents during each shift, confirming their well-being through physical checks.
  • Observe, report, and document any changes in residents’ conditions, including skin integrity, behavior, alertness, weight, diet, and activity participation.
  • Attend daily Cross Over meetings led by the lead Care Manager.
  • Promptly notify supervisors regarding any increase in resident care needs.
  • Communicate any alterations in residents’ conditions to supervisors.
  • Respond immediately to security systems and residents’ call bells, taking necessary actions such as resetting as required.
  • Provide a warm welcome to guests, family members, residents, and team members.
  • Manage phone inquiries in a timely, courteous, and professional manner.
  • Assist with continence management and ensure proper disposal of related products to maintain hygiene.
  • Engage and support families as a resource as necessary.
  • Help promote resident independence and safety while upholding their dignity in accordance with Resident Rights and Sunrise’s Principles of Service.
  • Participate in and support resident activities of daily living (ADLs), encouraging residents to engage in activities as noted in their ISP.
  • Foster engagement in life skills and other enriching activities for residents throughout the day, particularly in reminiscence settings.
  • Approach residents with empathy, respecting their unique communication styles and fundamental needs.
  • Maintain a warm and positive atmosphere, contributing to a calm environment.
  • Adhere to established safety regulations at all times.
  • Practice exemplary Standards of Care regarding cleanliness, hygiene, and health.
  • Facilitate daily resident activities.

Risk Management and General Safety

  • Collaborate with the community team to ensure compliance with national and provincial occupational health and safety regulations, promoting Risk Management programs and policies.
  • Always follow safety procedures, including the use of Personal Protective Equipment (PPE), and maintain familiarity with fire safety protocols and hazardous material guidelines.
  • Report all accidents or incidents immediately.
  • Identify and report unsafe conditions or equipment promptly.
  • Ensure that cords and equipment do not obstruct exits and comply with fire codes.
  • Understand and implement procedures for the safe handling and disposal of hazardous materials and infectious waste.
  • Report exposure to potential health risks immediately.
  • Safely store and monitor oxygen tanks.

Housekeeping and Laundry Services

  • Maintain the cleanliness of assigned residents’ rooms, including making beds, emptying trash, and organizing belongings.
  • Manage residents’ laundry as indicated in their ISP.
  • Maintain cleanliness of dining room linens.
  • Complete assigned housekeeping duties consistently.
  • Ensure common areas are presentable at all times.

Dining Service

  • Serve meals in the dining room and assist with dining operations as designated.
  • Foster a pleasant dining experience for residents during all meals.
  • Aid in the setup and cleanup of the dining area as required.
  • Participate in pre-meal briefings.
  • Adhere to diet specifications outlined in the ISP and Confidential Diet Board.
  • Document any changes in residents’ dining habits accurately.
  • Address dining needs of residents and guests while maintaining Sunrise hospitality standards.
  • Provide room service as necessary.
  • Ensure compliance with food safety protocols.

Specific Responsibilities for the Reminiscence Neighborhood Care Manager

  • Engage residents in life skills and enriching activities as mentioned in the ISP.
  • Encourage resident participation in afternoon social gatherings.
  • Incorporate the philosophy of “Creating Pleasant Days” into daily routines.
  • Integrate residents’ life skills into their activities.
  • Introduce diverse multi-sensory experiences throughout the residents’ day.
  • Participate in monthly correspondence with residents and their families.

Training and Contributing to Team Success

  • Collaborate effectively as part of a team, aligning with shared goals.
  • Demonstrate adherence to the Team Member Credo in daily interactions.
  • Commit to serving residents and guests in alignment with our Principles of Service.
  • Engage in programs aimed at enhancing customer and team member engagement.
  • Attend scheduled meetings, including Town Halls and departmental meetings.
  • Partake in training sessions led by Coordinators using structured mini-modules.
  • Follow all infection control protocols and safety procedures as stipulated.
  • Maintain compliance with required training and state regulations applicable to this role.
  • Undertake additional duties as assigned.
