JOB OVERVIEW
The Care Manager, also known as the Designated Care Manager, is entrusted with delivering the highest quality of care and services to a designated group of residents and their families within our assisted living, long-term care, and reminiscence neighborhoods. Responsibilities include providing hands-on care and support, as delineated in each resident’s Individualized Service Plan (ISP), while ensuring a safe and comfortable home-like atmosphere. The Care Manager is committed to embodying Sunrise Senior Living’s mission to “champion quality of life for all seniors,” adhering to all applicable federal, state/provincial, and local laws, as well as Sunrise’s standards, policies, and procedures.

RESPONSIBILITIES & QUALIFICATIONS
Essential Duties

Resident Care

  • Thoroughly review and document relevant information about residents in the Daily Log.
  • Engage in the formulation and monthly updates of Individualized Service Plans (ISPs).
  • Manage designated responsibilities and ensure regular physical checks on residents.
  • Monitor and record changes in residents’ conditions, including skin health, behavior, and participation in activities.
  • Participate in daily Cross Over meetings led by the Care Manager.
  • Timely notify supervisors regarding any increases in residents’ care needs or changes in their conditions.
  • Responsively address security system alerts and resident call bells, ensuring prompt action.
  • Serve as a welcoming presence for guests, family members, and fellow team members.
  • Answer and direct phone calls courteously and professionally.
  • Assist with continence management, ensuring proper sanitation of resident suites and community facilities.
  • Communicate effectively with families, serving as a resource as required.
  • Uphold residents’ independence and dignity in accordance with Resident Rights and Sunrise Principles of Service.
  • Engage residents in activities of daily living (ADLs) and encourage participation aligned with their ISPs.
  • Facilitate life skills and enrichment activities, particularly in reminiscence care settings.
  • Approach all resident interactions with empathy, recognizing individual communication styles and needs.
  • Foster a warm, personal, and positive environment for all residents.
  • Adhere to established safety regulations and best practices in cleanliness and hygiene.
  • Organize and participate in daily activities for residents.

Risk Management and General Safety

  • Collaborate with the community team to ensure compliance with health and safety regulations.
  • Consistently practice safety protocols, including the use of Personal Protective Equipment (PPE) and adherence to fire and safety regulations.
  • Immediately report any accidents, incidents, or hazardous conditions.
  • Maintain a safe environment by ensuring equipment and hazards are properly managed.
  • Be familiar with and implement procedures for handling bloodborne pathogens and hazardous materials.
  • Ensure the safe storage and monitoring of medical oxygen tanks.

Housekeeping and Laundry Services

  • Maintain cleanliness in assigned resident rooms and common areas.
  • Manage the washing and folding of resident laundry as needed.
  • Complete housekeeping tasks assigned by supervisors.

Dining Service

  • Serve meals and help ensure a positive dining experience.
  • Assist with meal setup and cleanup.
  • Follow dietary guidelines provided in residents’ ISPs.
  • Monitor dining habits of residents, documenting any changes.

Specific Responsibilities for the Reminiscence Neighborhood Care Manager

  • Facilitate life skills and enriching activities tailored to each resident’s ISP.
  • Promote social interactions during communal activities.
  • Implement the ‘Creating Pleasant Days’ philosophy in residents’ daily routines.
  • Incorporate multi-sensory experiences into daily activities.

Training and Team Contribution

  • Engage actively as a team member, working towards shared goals.
  • Adhere to Sunrise’s Principles of Service in daily interactions.
  • Participate in engagement programs and regularly attend team meetings.
  • Comply with required training and infection control protocols.
