Your Tasks and Responsibilities:

As a Home Health Aide, your role centers on enhancing your patients’ daily lives by removing obstacles they may encounter. You will visit their homes to assess their well-being and deliver essential care, which includes but is not limited to:

• Monitoring patients’ conditions through observation of their physical and mental health, as well as their activity levels.
• Providing housekeeping and laundry services, shopping for necessities, preparing and serving meals and snacks, and running errands.
• Assisting with personal care, such as bathing, dressing, and grooming.
• Educating patients on self-care techniques, including the proper use of mobility aids and tools for personal hygiene.
• Coaching family members in effective caregiving practices, including safe lifting, turning, and repositioning techniques, along with guidance on nutrition and housekeeping.
• Documenting patient information meticulously and communicating any changes or concerns to the nursing supervisor.
