JOB OVERVIEW
The Care Manager, or “Designated Care Manager,” plays a pivotal role in delivering the highest standard of quality care and services to a designated group of residents and their families within our assisted living, long-term care, and reminiscence neighborhoods. The Care Manager provides personalized care and emotional support as outlined in each resident’s Individualized Service Plan (ISP), all while fostering a safe and welcoming home-like atmosphere. This role is essential in embodying Sunrise Senior Living’s mission to “Champion Quality of Life for All Seniors,” adhering to all applicable federal, state, provincial, and local laws and regulations, as well as Sunrise’s own standards, policies, and procedures.

RESPONSIBILITIES & QUALIFICATIONS
Positions are available on all shifts.

Essential Duties
As part of the Sunrise team, supporting our mission, principles of service, and core values is crucial to this role. Our foundational belief centers on the inherent value of human life. Key responsibilities include, but are not limited to:

  • Resident Care

    • Review and document pertinent information about residents in the Daily Log.
    • Contribute to the development and monthly updates of ISPs.
    • Manage designated assignments and oversee a specified group of residents, ensuring their safety and well-being throughout the shift.
    • Observe, report, and document changes in residents’ conditions, such as skin, behavior, alertness, weight, diet, and activity participation.
    • Attend daily Cross Over meetings convened by the lead care manager.
    • Notify supervisors and the resident care director of any increased care needs.
    • Respond promptly to security alerts and resident call bells.
    • Communicate professionally and courteously with guests, family members, residents, and team members.
    • Assist with continence management while maintaining hygiene standards within resident suites and common areas.
    • Engage with families and serve as a resource as needed.
    • Support residents in maintaining their independence and dignity while adhering to Resident Rights and Sunrise Principles of Service.
    • Assist residents with activities of daily living (ADLs) and encourage participation in activities as specified in the ISP.
    • Foster engagement in life skills and enrichment activities throughout the day in reminiscence.
    • Cultivate an atmosphere of warmth and positivity while ensuring compliance with safety protocols and hygiene standards.
    • Actively participate in daily activities with residents.
  • Risk Management and General Safety

    • Collaborate with the community team to ensure compliance with national/provincial health and safety regulations and promote Risk Management initiatives.
    • Adhere to safety protocols and report any accidents or unsafe conditions immediately.
    • Maintain an organized environment, ensuring compliance with fire codes and safety practices regarding hazardous materials.
    • Monitor the proper storage and use of oxygen tanks.
  • Housekeeping and Laundry Services

    • Maintain cleanliness in resident rooms and common areas.
    • Handle residents’ laundry as needed.
    • Complete assigned housekeeping tasks diligently.
  • Dining Service

    • Serve meals and facilitate a pleasant dining experience.
    • Assist with dining room operations, including setup and cleanup.
    • Observe and document any changes in residents’ dining habits, ensuring adherence to prescribed diets.
  • Specific Responsibilities for the Reminiscence Neighborhood Care Manager

    • Engage residents in life skills and activities as outlined in the ISP.
    • Incorporate “Creating Pleasant Days” into daily routines and blend multi-sensory experiences into residents’ days.
    • Facilitate letter writing initiatives with residents and their families.
  • Training and Contributing to Team Success

    • Actively participate in team meetings and training sessions.
    • Uphold compliance with infection control protocols and maintain required training standards.
    • Complete all other duties as assigned.

Experience and Qualifications

  • Prior experience working with seniors is preferred.
  • A genuine desire to serve and care for seniors.
  • Ability to make informed decisions in the best interest of residents.
