Your Tasks and Responsibilities:

In this role, you will work to eliminate barriers in your patients’ daily lives. Your responsibilities will include visiting patients’ homes to provide essential care, which may encompass:

  • Monitoring patients’ physical and mental conditions, as well as their intake and output.
  • Offering housekeeping and laundry services, shopping for groceries and household necessities, preparing and serving meals and snacks, and running errands.
  • Assisting with personal care, including bathing, dressing, and grooming.
  • Empowering patients to care for themselves by instructing them in the use of mobility aids, specialized utensils for eating, and personal hygiene techniques and equipment.
  • Supporting family members in caregiving by teaching safe techniques for lifting, turning, and repositioning the patient, as well as advising on nutrition and household cleanliness.
  • Documenting patient information through journal entries and alerting nursing supervisors to any changes or unusual conditions.
