JOB OVERVIEW

The Night Supervisor plays a pivotal role in monitoring the health and well-being of our residents while providing essential leadership support and hands-on assistance with resident care and services. Key responsibilities encompass promoting residents’ overall well-being, facilitating medication administration training and monitoring, maintaining effective communication with resident support services and families, ensuring consistent documentation of quality care and services aligned with each resident’s Service Plan (SP), and upholding clinical quality. RESPONSIBILITIES & QUALIFICATIONS

Essential Duties

Coordination of Health Needs

  • Identify ongoing care and service needs of residents to promote optimal resident care outcomes.
  • Ensure thorough evaluation, follow-through, and documentation for residents experiencing changes in condition or healthcare needs.
  • Assess, monitor, and follow up with residents following any fall, accident, or incident, documenting and reporting as directed.
  • Maintain accurate and timely documentation in residents’ electronic health records.
  • Collaborate with the neighborhood coordinator to communicate with residents’ families and/or responsible parties regarding assessed needs and outcomes of monthly wellness visits.
  • Contact residents’ attending physicians as necessary or upon family request.
  • Assist in maintaining wellness files in accordance with Sunrise policies and relevant regulations.
  • Provide clinical support and assistance to community team members as required.

Quality Assurance and Regulatory Compliance

  • Strive for excellence in the delivery of care and services.
  • Demonstrate knowledge in key quality improvement areas, including the Resident-Centered Care Model and relevant Quality Indicators.
  • Actively participate in Sunrise Quality Assurance & Clinical Meetings.
  • Uphold knowledge of essential regulations governing resident care and services, including Residents’ Rights and the Resident Assessment process.
  • Support the RCD and other community team members in preparation for regulatory surveys and review Physician Reports for new residents as directed.
  • Order medications and equipment as instructed by the RCD.
  • Implement and monitor documentation related to resident clinical and health and wellness care.

Medication Programs

  • Regularly monitor each resident’s medication and treatment profile to ensure accurate administration and documentation.
  • Accurately transcribe and update physician orders in the resident’s electronic health record.
  • Demonstrate knowledge of the Six (6) Rights of Medication Pass (“Right” resident, medication, dosage, time, route, right to refuse).
  • Serve as the liaison to pharmacy services to ensure effective medication management.
  • Maintain organization and cleanliness in the medication room and treatment carts, monitoring for expired medications.
  • Ensure proper receipt, storage, inventory, and security of controlled substances.

Resident Care

  • Respond promptly and professionally to requests from residents, families, and visitors.
  • Maintain confidentiality of all resident information.
  • Review, update, and document changes in residents’ physical and/or behavioral conditions, ensuring appropriate updates to service plans.
  • Manage staffing for assigned shifts, taking attendance and addressing call-offs/no shows.
  • Ensure care managers are aware of their responsibilities during shifts and participate in crossover meetings for information sharing.
  • Carry resident group assignments during overnight shifts and manage unfilled assignments as needed.
  • Respond immediately to security system alerts and resident call bells, taking necessary actions.
  • Safely store oxygen tanks, exchange them when empty, and monitor prescribed levels.

Risk Management and Safety

  • Collaborate with the community team to ensure compliance with occupational health and safety regulations, Risk Management policies, and applicable safety standards.
  • Adhere to safety procedures, including the proper use of Personal Protective Equipment (PPE) and risk management protocols.

Infection Control

  • Understand and follow the Sunrise Infection Control & Prevention Program.
  • Provide training as needed to support infection control practices.
  • Handle hazardous exposure according to established guidelines and report incidents immediately.

Training and Contributing to Team Success

  • Actively participate in team initiatives and work toward collective goals.
  • Support the onboarding of new team members through shadowing and skills demonstrations.
  • Exhibit commitment to the Team Member Credo in daily interactions.
  • Engage in resident and team member engagement initiatives, including surveys and improvement workshops.
  • Attend regular meetings as directed and maintain compliance with required training.

Experience and Qualifications

  • Graduate of an accredited nursing program.
  • Current licensing as a Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN/LVN).
  • Minimum of one year of experience in home health, assisted living, hospital, or long-term care settings.
  • Knowledge of nursing practices related to the health and wellness of the elderly population.
  • Familiarity with applicable regulations and the ability to delegate tasks based on team members’ skills and roles.
  • Proficient in Microsoft Office applications and Sunrise systems, with the adaptability to learn new software.
  • Flexibility to work weekends and variable hours.
